Key Responsibilities:

• Engage with patients to understand their care needs and address healthcare concerns
• Collaborate with medical practice staff and Case Management teams to deliver comprehensive care
• Accurately document patients' medical history and issues in electronic health records (EHRs)
• Meet assigned Key Performance Indicators to drive patient outcome improvements
• Utilize various communication methods to effectively engage high-risk patients

Requirements:

• Certified or Registered Medical Assistant or certified EMT
• Proficiency in medical terminology and primary care physician practices
• Experience with electronic health records (EHRs) and care management analytics
• Exceptional written, visual, and verbal communication skills
• Ability to work independently as part of a patient's care team under supervision

Preferred Qualifications:

• Familiarity with EHR systems such as EPIC, Athena, or Cerner
• Experience in motivational interviewing and behavioral change management
• Strong critical listening skills and empathy in patient coordination
• Ability to manage multiple tasks while maintaining attention to detail
• Proven success in a remote work environment
