Housing and Community Support Specialist

About the position The Housing and Community Support Specialist at Boston Medical Center's Living Well at Home Program (LWAH) is responsible for providing high-quality housing case management services to clients facing homelessness or housing instability. This role involves engaging with high-risk patients, advocating for their needs, and assisting them in obtaining and maintaining independent housing. The specialist will work collaboratively with an interdisciplinary team to develop care plans, facilitate access to resources, and monitor patient progress, ensuring a patient-centered approach to support their housing and health goals. Responsibilities • Provide case management services to high-risk patients with behavioral health diagnoses experiencing long-term homelessness. , • Engage and enroll complex patients into services, advocating for their needs. , • Support patients in becoming 'housing-ready' and assist in identifying and obtaining housing opportunities. , • Develop interdisciplinary care plans based on identified patient needs. , • Facilitate access to social service resources and monitor patient progress. , • Problem-solve with patients to enhance access to housing and community-based supports. , • Conduct community-based outreach and home visits to support patients across Greater Boston. , • Collaborate with various stakeholders, including housing providers and care teams, to address patient needs. , • Assist patients in overcoming barriers related to housing, health, and social services. , • Maintain accurate documentation and compliance with data entry requirements. Requirements • Bachelor's degree in a behavioral health or related field, or two years of relevant work experience, or lived experience of homelessness or behavioral health conditions. , • Minimum of 2 years prior healthcare, public health, or social services work in a community-based setting. , • Basic knowledge of housing systems and community resources, particularly in the Boston area. , • Outstanding interpersonal skills and ability to communicate professionally with diverse populations. , • Proficiency in Microsoft Office applications and data entry. Nice-to-haves • Fluency in Haitian Creole or Spanish is preferable. , • Experience working with individuals impacted by mental illness, substance use disorder, and/or chronic health conditions.
