Oncology Nurse Navigator - Breast Specialist Clinic - Full-time - Days

The Oncology Nurse Navigator (ONN) serves as an expert clinician guiding patients and families through cancer care, survivorship, and follow-up. Using NCCN guidelines, the ONN coordinates multidisciplinary care, facilitates access to resources, and develops survivorship care plans tailored to patient goals. The role ensures accreditation standards with COC and NAPBC, addresses healthcare disparities, and promotes health through education, advocacy, and community referrals. Requirements:

  • Graduation from an accredited school of nursing, BSN strongly preferred.
  • Licensed by the Louisiana State Board of Nursing
  • Oncology Nursing Certification (OCN) is preferred or completed within 2 years
  • Oncology Nurse Navigation Certification (ONN-CG) is preferred or completed within 3 years
  • Minimum 5 years of oncology, navigation, palliative care, end of life or hospice experience Responsibilities of Clinic Nurse Practitioner include, but are not limited to:
  • Assess patient, family, and caregiver needs across the care continuum, identifying barriers, concerns, and resources required.
  • Educate patients and families on diagnosis, treatment, side effects, symptom management, and available support services.
  • Collaborate with providers to coordinate treatment plans and ensure timely, seamless communication between patients, caregivers, and the care team.
  • Facilitate access to multidisciplinary and community resources, including financial, psychosocial, nutritional, genetic, and rehabilitation services.
  • Support patients in overcoming barriers to care, promoting self-advocacy, and making healthy lifestyle and behavior choices.
  • Conduct survivorship and risk assessments; develop and deliver individualized survivorship care plans in collaboration with physicians.
  • Guide patients through transitions from active treatment to survivorship and from specialty to primary care providers.
  • Build partnerships with community organizations and lead outreach activities to expand education and support opportunities.
  • Track and evaluate program outcomes, using data for continuous quality improvement and reporting to committees and regulatory agencies.
  • Applies evidence-based practice in alignment with NCCN guidelines to guide patient care and navigation activities.
  • Ensures compliance with accreditation standards by supporting data collection and reporting for COC and NAPBC.
  • Maintain professional patient relationships, actively participate in multidisciplinary meetings, and pursue ongoing professional development.
