Responsibilities:

As an LPN Home Health professional, you will be responsible for:

• Verifying the plan of care with the RN prior to each visit
• Collaborating with physicians and RNs to ensure patient needs are being met
• Teaching patients and caregivers self-care techniques
• Checking vital signs and reviewing medications
• Performing specialized procedures ordered by physicians
• Preparing equipment and materials for treatments using aseptic technique
• Completing documentation in a timely and accurate manner

Qualifications:

To be considered for this role, you must have:

• One year of clinical LPN experience
• Current LPN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance
