Adecco Medical & Science has an outstanding opportunity for a Case Manager to join an impactful company in San Jose, CA. This is a full-time contract to hire position. Contract: Temp to Hire Schedule: Monday-Friday 8:30AM-5:30PM Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) strongly preferred. Primary Responsibilities: • Engages and enrolls community members eligible for ECM per care plan protocols • Assesses client need areas • Develops an individualized care plan with each client that is updated as needed • Coordinates care for clients with a multidisciplinary clinical team and outside agencies. • Facilitates referral linkages to community resources and performs case management services. • Assist clients with the development of social and community support systems. • Develops effective working relationships with agencies, organizations and providers to advocate for clients. • Documents and maintains clinical records per appropriate standards and regulations. • Conducts outreach in diverse community settings. • Participate in clinical supervision, consultations, and trainings. • Provide translation services as needed. • Complete other duties and related projects as assigned. Required Qualifications: • Bachelor’s degree in psychology or a related field, or equivalent training and experience that would provide the ability to perform the duties described above. • One year or more of case management or related experience. • Fluent in English and one of the Santa Clara Country threshold languages (Spanish, Tagalog, Vietnamese, Mandarin, Cantonese) preferred • Trauma informed training preferred Preferred Qualifications: • Familiarity with community-based resources. • Excellent problem-solving and organizational skills. • Understanding of cultural issues, community resources and family dynamics. • Ability to communicate well with people of diverse cultural, professional, and experiential background. • Ability to establish and maintain effective work relationships as part of a multidisciplinary team.
