Connected Care Nurse

Job Category: Registered NurseJob Description:Evening shift 3pm- 11pmJob Summary:Administers care and education to patients and families through a virtual platform. Provides resources, education and leadership to the nursing caregivers and communicates interdepartmentally with the care team. The Connect Care Nurse will be located on the Good Samaritan campus and be an integral part of the patient care team.Essential Job Duties: * Assess patient need for self-care instructions related to admission diagnosis or chronic disease process.

• Collaborates with care team to develop the plan of care for patients requiring education and/or intervention.
• Initiates instruction and promotes self-care.
• Implements strategies to provide patient, significant other, and staff participation in health promotion.
• Utilizes age specific criteria to provide education.
• Documents in designated sections of computerized record (i.e. education, discharge).
• Maintains communication both intra and interdepartmentally for continuity of care and patient education.
• Seeks educational resources and creates learning experiences to enhance and maintain current knowledge and skills for his/her competence in nursing practice and individual professional growth.

Secondary Job Duties That May be Reassigned: * Performs other duties as needed or assigned.Job Specifications: * Education: Graduate from an accredited school of nursing. Certification preferred.

• Experience: Graduate of RN Program

Five years of nursing practice with teaching experience preferred.Two years of experience in perspective specialtyComputer skills including Microsoft, PowerPoint, Word, Excel * Licensure: Current Registered Nurse licensure in the state of IndianaBasic Life Support certificationPerformance Requirements: * Mental:Able to interpret data, synthesize information, critical thinking and problem solving. * Physical:Hand and eye coordination and manual dexterity for keyboard operation. Ability to visually assess clinical practice, patient conditions and to communicate with patients, peers, ancillary staff, physicians, students and patient's families. * Skills & Abilities:Computer skills including Microsoft PowerPoint, Word, Excel. Specialized clinical management and analytic skills. Communications skills - ability to communicate clearly and empathetically with patients over virtual platforms, ensuring that patients understand instructions and feel supported. * Working Conditions:Clean, well-lighted area.Reports to:A.
