The LVN Case Manager, in partnership with the Medical Management team and Physician Leadership, will perform case management, coordination of care and medical management consulting within an interdisciplinary team. This includes collaborating with the Northwell Hospital System and Optum partners on complex patient care coordination. Position in this function provides health education, coaching and treatment decision support for members. Required Qualifications:

  • Current unrestricted NY LVN license
  • 2+ years of experience in a physician's office, clinical or hospital setting
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
  • Proficient computer skills to work efficiently with electronic medical records

Preferred Qualifications:

  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting
  • Case management experience

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
