Key Responsibilities

As a Home Health Occupational Therapist, you will collaborate with the Case Manager to provide skilled therapeutic services to patients in their homes. Your role will include: • Conducting evaluations and addressing patient needs to promote safe and productive living. • Performing home safety assessments and making necessary modifications. • Implementing medication management strategies. • Assisting with wheelchair seating and positioning. • Administering pain management techniques. • Developing falls prevention programs. • Managing chronic diseases and dementia. • Training families and caregivers. Required Skills To excel in this position, you will need: • An active and unrestricted CA Occupational Therapist License. • Current BLS/CPR Certification. • A valid CA Driver’s License. • Knowledge of local community resources. • An understanding of individual development, human behavior, and cultural influences in healthcare. • Knowledge of homecare regulatory guidelines (preferred). • Experience in home health care (highly preferred). • Familiarity with high-acuity patients (a plus). • The ability to work effectively within an interdisciplinary team (highly desired). • Fundamentals of case management (highly desired). • Self-direction, empathy, and critical thinking skills. • Strong interpersonal and leadership abilities. • Excellent organizational skills. Qualifications • A degree in Occupational Therapy from an accredited institution is required. • Prior experience in home health care is highly preferred. • Experience with high-acuity patients and diverse populations is valued.
