About the position The Advanced Illness Management Transitions team at SCAH-Sutter Care at Home is seeking a dedicated Registered Nurse (RN) for a hybrid position that combines 50% remote tele support with 50% direct patient care in the Central Valley, specifically in the Roseville and Modesto areas. This role is crucial in providing comprehensive in-home nursing care, which includes not only direct patient care but also patient and family education, as well as managing transitions of care. The RN will be responsible for delivering holistic and individualized care tailored to the unique needs of each patient in their assigned area. In this position, the RN will develop, implement, and manage an interdisciplinary plan of care that takes into account various factors such as psychological, sociocultural, spiritual, economic, and lifestyle considerations. The role requires fostering and maintaining collaborative relationships with patients, their families, physicians, and other healthcare providers through effective and timely communication. Adherence to policies, industry standards, best practices, and applicable laws and regulations is essential to ensure a high-quality and reliable patient experience. The ideal candidate will have a strong background in nursing, with a focus on home health or ambulatory settings, and will be expected to demonstrate proficient knowledge of nursing theories, medical terminology, anatomy, and physiology. The RN will also need to possess excellent communication skills to convey sensitive information clearly to diverse audiences, including those without medical backgrounds. This position is full-time, with a day shift schedule, and does not require weekend work. Responsibilities • Accountable for the assessment, coordination, delivery, and evaluation of in-home nursing care. , • Provide direct patient care and patient/family education. , • Manage transitions of care effectively. , • Develop and implement an interdisciplinary plan of care that considers various patient factors. , • Foster collaborative relationships with patients, families, and healthcare providers. , • Adhere to policies, industry standards, and applicable laws to promote quality patient experiences. Requirements • Graduate of an accredited school of nursing. , • Registered Nurse (RN) license in California. , • Basic Life Support (BLS) certification. , • California Driver's License and valid auto insurance. , • 5 years of experience as an RN in a home health or ambulatory setting. , • Proficient knowledge of nursing theories, concepts, and practices. , • Working knowledge of medical terminology, anatomy, and physiology. , • Strong written and verbal communication skills. , • Proficient in Microsoft Office Suite and Electronic Health Records. , • Excellent time management and organizational skills. Nice-to-haves • Experience in a home health or ambulatory setting is preferred. , • Knowledge of safety precaution policies and best practices regarding patient care.
