MDS Nurse (RN/LPN) - Cassia - Eagle, CO - work from home job Company: Cassia Job description: Join our team at Castle Peak! We are looking for an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Part-Time MDS Coordinator. In this role, you will manage the clinical care of residents by assigning different tasks to nurses within the department at one of our locations. Our ideal candidate has prior LTC experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills. This is a Part-Time, benefit eligible position working 20 to 30 hours per week. It is mostly remote work with 3 to 4 site visits a month. Pay Range: $30 to $50 / hour depending on experience. MDS Coordinator responsibilities: Oversee and complete MDS and care planning. Manage and assign tasks to nursing staff on the assigned unit. Complete all documentation for RAI and MDS assessments and observations. Develop initial and temporary care plans. Assist Case Manager with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage. Document and maintain paperwork, referral records, and reports. Work in collaboration with Admissions staff, Case Manager, and Supply Coordinator. Display a courteous attitude and respect for all residents, families, and staff. Will be part of the management team and do MOD rotation. Assist with floor coverage as needed. Perform additional tasks as needed. MDS Coordinator skills and requirements: RN or LPN license in the state of Colorado required. Minimum of 1+ years nursing experience required. Experience with MDS preferred. Proficient in computer software.
