Remote Nurse Case Manager Opportunity

Job Overview We are currently seeking a committed and proactive Registered Nurse (RN) to become a valued member of our team in a hybrid capacity, combining remote work with essential in-person visits. This role presents an exceptional opportunity to create a significant impact within the healthcare community while enjoying the benefits of a flexible schedule. Key Responsibilities • Manage patient care and attend necessary medical appointments. • Coordinate hospital visits, procure medical equipment, arrange home modifications, and facilitate additional supportive services. • Operate independently, maintaining a flexible schedule while traveling locally as required. • Deliver outstanding care and support to patients, ensuring optimal treatment outcomes. • Engage in travel for in-person visits, which will comprise 50%-75% of your time. Required Skills • Active Oregon RN License. • Valid Oregon Driver's License along with Auto Insurance. • Proficiency in Microsoft Word and Excel. • Bilingual proficiency in Spanish is advantageous. • Strong ability to work autonomously while also functioning as part of a collaborative team. Qualifications • A minimum of 1 year of Nursing Experience (preferred). • A minimum of 1 year of Case Management Experience (preferred). • Active RN License is required. • Openness to travel locally for in-person visits (50%-75%).
