[Remote] Cardiac and Pulmonary Rehab Telehealth Physician (Internal Medicine)
Note: The job is a remote job and is open to candidates in USA. Carda Health is building the world’s first patient-centered virtual heart and lung platform, focusing on cardiac and pulmonary rehab. They are seeking an Internal Medicine or Family Medicine physician to provide high-quality clinical care, perform patient evaluations, and coordinate care plans in a telehealth setting.
Responsibilities
- Join our medical group as an independent contractor
- Provide high-quality clinical care utilizing best practices and policies set forth by Carda’s clinical leadership team
- Perform asynchronous or synchronous visits with patients on a schedule that suits your schedule
- Perform health histories, evaluate patient records, diagnose, and refer to our programs when appropriate
- Provide lifestyle recommendations to patients related to specific goals
- Input notes and all other relevant information into our EMR
- Communicate and partner with other members of the patients’ care team to properly coordinate care and oversee patient care plans on an ongoing basis
- Join in occasional meetings and participate in a QA program
Skills
- Internal Medicine or Family Medicine physician
- Board certified in Family Medicine, Internal Medicine, Cardiology, Pulmonology
- Ideally licensed in five or more of the states. Priority states include: Florida, Pennsylvania, Virginia, Texas, Maryland, Illinois, Ohio, Michigan, Washington, Massachusetts, North Carolina, California
- Clean, or relatively clean record to facilitate ease of health plan credentialing
- Not currently opted out of Medicare
- Willing to complete credentialing applications in a timely matter
- Expertise in the management of individuals with cardiac and pulmonary pathophysiology
- Cardiopulmonary training in basic life support or advanced cardiac life support
- Passion for delivering high quality care to patients
- Experience with care in a virtual setting and a desire to learn and influence new technologies
- Highly organized with the ability to create, adhere to, and scale repeatable processes
- Licensed In The Following States: Alabama, Arkansas, Georgia, Iowa, Louisiana, Mississippi, South Carolina, West Virginia
- Willing to obtain additional licenses by mutual consent and with financial support
