Job Description

The Clinical Intake Coordinator, LPN facilitates the Home Health & Hospice referral process by receiving referrals from multiple referral sources within and without the Saint Luke's Health System. We are seeking a compassionate and dedicated Clinical Intake Coordinator/LPN to join our supportive environment where patients and staff are valued. Monday – Friday, 9 a.m.-5:30 p.m. One remote on-call shift required every 5-6 weeks. Rotating (remote) weekend and holiday requirement. Hybrid - Employees are required to work on-site 3–4 days per week, with the option to work remotely 1–2 days per week upon completion of orientation. Remote days are scheduled on a rotating basis to ensure adequate office coverage. Work arrangements are subject to change based on business needs. Determines need for additional information from referral sources Communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. Prioritize and triage hospice referrals. Ensure regulatory compliance is met during initial screening and intake process. Missouri and Kansas LPN License Required. Why Saint Luke’s?   • Missouri and Kansas LPN License Required.
