What to Expect (Job Responsibilities)

As a Field Community Health & Social Impact Specialist, you will play a critical role in ensuring the quality and effectiveness of our healthcare services. Your responsibilities will include:

  • Conducting medical management activities to ensure quality patient care
  • Collaborating with healthcare providers to coordinate and manage patient care
  • Reviewing medical records and treatment plans to ensure compliance with regulations
  • Providing guidance and support to patients and their families regarding medical treatment options
  • Participating in medical rounds and case conferences to discuss patient care strategies

What is Required (Qualifications)

To be successful in this role, you will need to possess the following qualifications:

  • Bachelor's degree in Nursing or related field
  • Active RN license in the state of Maryland
  • Minimum of 3 years of experience in medical management or healthcare coordination
  • Strong knowledge of medical terminology and procedures
  • Excellent communication and interpersonal skills

How to Stand Out (Preferred Qualifications)

While not required, the following qualifications will make you a strong candidate for this role:

  • Certification in Case Management (CCM)
  • Experience working with electronic medical records (EMR) systems
  • Knowledge of Medicare and Medicaid regulations
  • Bilingual in English and Spanish
  • Experience in a managed care setting
