Job Overview

This position is for a Licensed Practical or Vocational Nurse to provide skilled, in-home care to patients under the supervision of a Registered Nurse. The role involves delivering high-quality, documented care in a compassionate and empathetic manner, ensuring all activities are in compliance with physician orders and established care plans. This is a night shift position.

Key Responsibilities

• Provide and coordinate quality patient care in compliance with a physician's orders.
• Observe and assess patient conditions and needs continuously, reporting any changes to the supervisor or physician.
• Document all activities, assessments, and nursing actions in a timely and confidential manner.
• Participate in the implementation and updating of the patient’s nursing care plan.
• Take appropriate nursing actions to achieve expected patient outcomes and prevent or reduce risk.
• Educate patients and their families about disease processes, self-care techniques, and prevention strategies.
• Comply with all policies, state and federal regulations, and professional standards.
• Maintain a commitment to infection control measures and professional development.

Requirements

• A current, unrestricted license as a Licensed Practical or Vocational Nurse in the state of practice.
• A graduate of an accredited school of nursing.
• Current CPR certification with a hands-on component.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing policies.
• A recent TB skin test (within the last 12 months).
• Reliable transportation.
• Compliance with all health-related requirements, including required vaccinations and testing.
• Physical ability to lift up to 50 pounds, reposition patients, and move equipment.

Preferred Qualifications

• Six months of recent experience as a Licensed Nurse in a clinical care setting is preferred, but not required.
• Experience in home health care.
• Depending on the state of practice, one year of experience may be required (e.g., South Carolina, California, Louisiana).
